Enrollment Form

The Curious Caterpillars Learning Center admits ages 6 weeks-5 years and makes available to them its advantages, privileges and curriculum without regard to race, color, sex, religion, national origin, sexual orientation or disability.

TCCLC has a published admission policy that is made available to all applicants and parent(s)/guardian(s) as part of the admission process. The policy gives the admission criteria, as well as a description of the entire admission process, center and family expectations.

This application form must be completed and submitted to the TCCLC Director of Education. In addition to this application form, the applicant's current childcare provider will include academics if applicable, payment standing, attendance record and discipline/conduct record.
